FSU students, faculty, and staff may schedule an appointment for any vaccination clinic on the calendar below to receive their COVID-19 vaccination. Please check your FSU email account for the entry code and instructions.

COVID-19 Vaccine

Florida State University is currently offering COVID-19 vaccinations at the Health and Wellness Center (4th Floor) for students, faculty, and staff. Vaccinations will be administered in accordance with Centers for Disease Control and Prevention guidance.

At this time, the university is only administering the Pfizer COVID-19 vaccine. The Pfizer vaccine is fully approved by the U.S. Food and Drug Administration for use in individuals 16 and older and is currently under Emergency Use Authorization for use individuals ages 5-11 and 12-15. Please note, minors must be accompanied by a parent or legal guardian in order to receive the vaccine. The university is not offering the vaccination for any individuals aged 5-11. Students, faculty, and staff who received their first Pfizer vaccination elsewhere may complete their series at the FSU COVID-19 Vaccination Clinic.

First and Second COVID-19 vaccination doses are not interchangeable. If you received a Pfizer or Moderna COVID-19 for your first dose, you should get the same product for your second dose. If you are in need of a second dose of Moderna or would prefer to receive the Johnson & Johnson COVID-19 vaccine, please visit vaccines.gov to find a provider near you.

Booster Pfizer COVID-19 vaccinations are authorized for individuals 12+ by the FDA and CDC for those who received the Pfizer COVID-19 Vaccine Series, and for individuals 18+ who received the Moderna COVID-19 Vaccine Series, or the Johnson & Johnson (Janssen) COVID-19 Vaccine and are past the recommended time frames from completion of the initial series. The Pfizer Booster Vaccination will be administered in accordance with the guidance from the FDA and CDC. For those who are eligible and would like to receive a Moderna or Johnson & Johnson (Janssen) COVID-19 Vaccine Booster, please visit vaccines.gov to find a provider near you.

Account Creation & Appointment Instructions

FSU students, faculty, and staff must "Create An Account" at PatientPortalFL.com in order to schedule your vaccination appointment. Once you have created an account, you will need to schedule your vaccination appointment and complete medical screening questions by clicking "Get Vaccinated" on the homepage of your account screen. Please check your FSU email account for the entry code and instructions. If you have issues creating an account or scheduling an appointment you may come to a scheduled clinic for assistance or contact CDR Maguire using the information below for more assistance.

Please review the calendar below for scheduled clinic locations, dates and times and be sure to bring your FSUCard with you. At this time, the University is not scheduling clinics that are open to the general population. If you are not a FSU student, faculty, or staff member and would like to get vaccinated please visit vaccines.gov to find a provider near you!
